Instructions

You must be 18 years or older to receive the COVID booster vaccine
1) You will be required to wear a mask for the duration of your appointment.
2) Please allow 45 minutes for your appointment time. You will be monitored for 15 minutes after receiving the vaccine.
3) Please bring your vaccine card with you to your appointment.
